How new residents are admitted to the program.
Who is eligible for placement at Rosehedge?
- Must have an AIDS defining illness
- Must require substantial or total assistance with three or more of the following:
- Eating
- Ambulation
- Body care
- Bathing
- Toileting
- Self-Medication
- Must have documented medical problems or cognitive impairment making it difficult or impossible to live independently
- Must be unable to maintain or coordinate his/her treatment plan
Summary of the admissions process
- A referral is made by the client's Case Manager or Social Worker
- Client is added to the wait list
- When a room becomes available, Rosehedge contacts the Case Manager of the client at the top of the list
- Case Manager submits a completed referral packet
- An intake screening is performed to determine whether the client is in need of the level of care we provide
- Home and Community Services performs an assessment to determine whether the client is eligible for financial assistance through the Department of Social and Human Services
- If eligible, the client is offered a room at Rosehedge
- If the client is not eligible or decides not to move in, we contact the Case Manager of the next person on the wait list
