Director of Clinical Services
POSITION CLOSES: Thursday, February 23rd or Until Filled
REPORTS TO: Executive Director
WORKING HOURS: 32 hours per week; participates in on call rotation
COMPENSATION: Salary: $ DOE. Medical, Dental Benefits and Vacation and Sick Time.
THE POSITION: The Director of Clinical Services provides oversight and guidance for the day-to- day operations of the Enhanced Assisted Living Program(EALP) ensuring the agency's mission is evident in every aspect of the program. This includes oversight of resident care, supervision of all staff, contact with funding sources, ensuring cross-program cooperation and consistency of services as a member of the Management Team, budget management, program planning and evaluation.
CLINICAL
- Directs and manages the day-to-day operation of the Enhanced Assisted Living houses; when needed provide nursing care
- Supervises the nursing personnel providing "intermittent nursing" care to the residents
- Guides and directs the nurses in their supervision of the home health aids providing care to residents 24/7.
- Works together with the clinical team, nurses, home health aids, social workers and support staff, to provide structure and process supporting each resident's needs
- Ensures that policies, procedures and guidelines are current and available as a resource to clinical staff
- Ensure clinical staff compliance with Organization's policies, procedures and guidelines.
- Hires, orients, and trains all EALP staff.
- Ensures compliance with OSHA/WSHA regulations, Enhanced Adult Family Home regulations, WACs, RCWs and the Landlord Tenant Law.
- Develops and tracks quality improvement goals and progress
- Interfaces with regulatory agencies as appropriate
- Ensures that documentation meets the standard requirements and provides for excellent communication among care givers, including home health aids, nurses, social workers, case managers and physicians.
- Works with the chef to assure resident meals and snacks are nutritious, satisfying and appealing incorporating special diet needs.
- Coordinate services with collaborating community agencies.
- Perform and oversee resident admission process and health evaluations.
- Participates in care conferences, transition planning and discharges.
- Maintains open communication with case managers, physicians and other agencies working with residents.
- Comply with Rosehedge Multifaith Works employment policies
- Provide individual counseling when needed
- Identify and resolve performance issues with a goal to help staff be successful and accountable
RESIDENT CARE/RESIDENT RELATIONS
- Meet with residents as needed, especially those struggling with the living environment
- Provide access and follow-through with the Resident Grievance Process.
- When needed, proceed with implementation of disciplinary Notices in conjunction with the social worker and case manager
HOUSING STAFF SUPPORT
- Respond to on-call needs of home health aides, social worker and nurses
- Meet with support staff on a regular basis (housing program coordinator, social worker, nurses, home health aides)
- Encourage self-care
- Meet with staff, as a group, regularly - minimally once a month.
- Complete 6 month and annual Performance Evaluations
- Ensure consistent and effective communication among and between staff.
- Administrative:
- Assist with program development, reporting, and evaluation.
- Attend all Staff, Agency, Management meetings.
- Attend all Ryan White and HOPWA Service Provider and Housing Provider Meetings.
- Ensure that all required reports to funders are submitted on time.
- Assist Development with grants.
- Completes monthly Board reports.
- Monitor progress on annual EALP Work Plan.
- Consult with attorneys when needed.
- Ensures that houses are well-maintained.
- Management Team:
- Provides peer support to all program directors.
- Assist in agency policy recommendations.
- Assist in strategic initiatives.
- Assist in the programmatic growth of the agency.
- Collaborates with the Shanti and CareTeam programs in their work with housing residents.
- Community Representative:
- Represents agency within the Tri-County HIV/AIDS Continuum of Care.
- Fosters relationships with collaborating agencies that provide resident services.
- Develop connections to outside services that support residents.
- Connects with other housing providers as a voice and advocate for people with HIV/AIDS.
- Engage in culturally-proficient approaches that result in increased resident engagement.
- Plan a yearly luncheon and house tours with providers and Board members.
ESSENTIAL QUALIFICATIONS
- Current licensure for registered nurse in the state of Washington, with minimum five years nursing experience
- Minimum five years clinical management/supervisory experience
- Evidence of ability to problem-solve and make independent decisions
- Recognizes own limitations and seeks assistance appropriately
- Evidence of knowledge of palliative care, symptom management and assessment skills
- Utilizes skill and judgment in applying basic principles of professional nursing
- Ability to handle emergencies
- Demonstrated ability to establish and maintain effective working relationships with coworkers, other health care professionals, residents/clients and families
- Presents self in a professional, confident and caring manner
PREFERRED QUALIFICATIONS
- Minimum two years of experience with HIV/AIDS, homeless population and mental health, chronic substance use populations preferred
- A demonstrated knowledge, experience, and interest in cultural, ethnic and spiritual diversity
- A developed personal system for self-care
- Willingness to make a long-term commitment and adapt to flexible working hours, including evenings and weekends

House Registered Nurse
POSITION CLOSES: Wednesday, February 8th or until filled.
REPORTS TO: Housing Director
COMPENSATION: Salary: $ DOE. Medical, Dental Benefits and Vacation and Sick Time.
WORKING HOURS: Mon - Fri
THE POSITION: The House Registered Nurse is the manager and supervisor for the entire House Team.
- The RN is responsible for;
- Resident admissions, conducting resident assessment, documentation, reevaluations and communication on behalf of all house residents.
- Working with primary health care providers to administer/ manage medications, assess effectiveness and side effects of medications and other treatments.
- Assessing the environmental safety for all residents and staff and for making/recommending any necessary changes.
- Establishing working relationships with individual Home Health Aides (HHA), as well as demonstrating the ability to oversee and evaluate individual HHAs.
- In collaboration with the Social Worker, developing and following the care plan for each resident and for providing guidance for residents and their support networks regarding the care plan.
- All changes and adjustments to the care plan are the co-responsibility of the RN and Social Worker, and include the documentation associated with all changes and plans.
- The RN assumes ultimate responsibility for the delivery of safe, adequate care for all House residents.
- As part of the supervisory responsibilities, the RN will manage all staff conflicts, facilitating resolutions when needed and consulting the clinical team for guidance in direction when necessary.
- As a member of the clinical supervisory team, the RN is responsible for attending all staff meetings, and providing support for all staff meetings and staff case conferencing as needed.
- The RN also provides support for residents with regard to pain management, symptom management, and medication management (administration, adverse reactions, therapeutic value, resident teaching); pathophysiology of HIV/AIDS diseases or other terminal/chronic illness; issues of death and dying; and normal and abnormal response to loss and grief.
- The RN is responsible for documenting all residents' conditions, recommendation for changes or referrals to other services and providing support to residents and their support systems regarding any changes or referrals.
- The RN will be in regular consultation with the clinical team to reevaluate all residents' care plans and suggest recommendations for change if necessary.
THE CANDIDATE:
Rosehedge/Multifaith Works seeks an ideal candidate who is passionate about AIDS care, housing services, drug and alcohol recovery and care, community engagement, and diversity. The ongoing work to determine what is best for each resident creates a continually changing dynamic within the relationships of the residents in each house and with housing volunteers and staff; flexible firmness is the key.
ESSENTIAL QUALIFICATIONS:
- Current licensure for registered nurse in the state of Washington
- Current BLS
- Current First AID/CPR
- Possess strong triaging and problem solving skills and makes sound, independent decisions. Evidence of ability to problem-solve and make independent decisions
- Recognizes own limitations within the scope of role and seeks assistance appropriately
- Possess strong assessment and symptom management skills
- Utilizes skill and judgment in applying basic principles of professional nursing
- Ability to handle emergencies and triage situations
- Ability to act in a supervisory capacity over Home Health Aides and other staff
- Demonstrated ability to establish and maintain effective working relationships with coworkers, other health care professionals, residents/clients and families
- Presents self in a professional, confident and caring manner
- Advanced Computer skills: Office 2010, with professional experience in database charting
PREFERRED QUALIFICATIONS:
- Experience with HIV/AIDS, homeless population and heavy emphasis on mental health, chronic substance use populations preferred
- A demonstrated knowledge, experience, and interest in cultural, ethnic and spiritual diversity
- A developed personal system for self care
- Willingness to make a long-term commitment and adapt to flexible working hours, including evenings and weekends
- Willingness to make a minimum one-year commitment to Rosehedge/Multifaith Works
